Service Overview
Basic Support Services
Basic Support Services are designed to offer fundamental assistance to individuals, ensuring their day-to-day needs are met effectively. These services include 24-hour emergency assistance for urgent support, adult companion services for social interaction and emotional support, homemaker services for essential household tasks such as cleaning and meal preparation, and night supervision services to provide safety and security during nighttime hours. Respite care, available in-home or out-of-home, offers temporary relief for primary caregivers, allowing them to take breaks while ensuring continuous, high-quality care for their loved ones.
Intervention Support Services
Intervention Support Services provide specialized support during critical times. Crisis respite services, available both in-home and out-of-home, are intended to offer immediate, intensive support to stabilize individuals experiencing crises or heightened needs. These services are designed to address urgent situations and facilitate a return to regular care routines with minimal disruption.
In-Home Support Services
In-Home Support Services focus on delivering personalized support within an individual’s home. This category includes individualized home supports to address daily living activities and foster independence. Specialized options are available, such as home supports with training to enhance skills and promote self-sufficiency, and family training to involve family members in the care process. The aim is to create a supportive home environment that encourages personal development and autonomy.
Employment Services
Employment Services are designed to help individuals achieve and sustain meaningful employment. Services include employment development to build job skills and secure job placements, employment exploration to assess interests and abilities, and employment support to provide ongoing assistance in maintaining employment. These services are aimed at facilitating successful job outcomes and promoting long-term employment stability.
Integrated Community Supports
Integrated Community Supports focus on helping individuals participate fully in their communities. These services assist with accessing community activities, resources, and services to promote social inclusion and engagement. Support is also provided for the development and maintenance of informal support systems, including transportation when necessary, to ensure individuals can actively connect with and contribute to their communities.

GETTING STARTED How to Get 245D Waivered Services

Obtain an HCBS Waiver
- Reach out to your local County Human Services Office to initiate the process. Request a MnCHOICES assessment to determine your eligibility for Home and Community-Based Services (HCBS) Waivers.

Work with a Social Worker
- Collaborate with a social worker to identify the HCBS Waiver that best suits your needs. Review and select from the available services within your chosen waiver.

- To get in touch with Ontime Health Care, call (651) 206-1897 or email info@ontimehealthcare.com. After we receive your request or referral from your social worker, we will arrange an intake meeting to discuss 245D Waivered Services. Following this, you can choose and engage your preferred Direct Support Staff.
